Mentee Application

Thank you for your interest in the FOY Mentorship Program. We are excited to have you participate. 

All applications will be reviewed on a rolling basis and each applicant will be contacted as to the status of their application.

PLEASE NOTE: You cannot save and return to complete this form. You may want to review the questions in advance so that you leave enough time to complete the application form. 

Information may be disclosed to Fostering Our Youth, Inc. background screening company. This information is protected under the Privacy Act of 1974. 

By signing and submitting this form, I agree to the following: 

1. Attend a brief mentee training before the program kick-off (TBD) 

2. Actively participate in events and programs sponsored by the FOY Mentorship Program. 

3. Keep in regular contact with my mentor according to the program guidelines. 

4. Notify the Peer Mentorship Program Coordinator if I am not satisfied with my mentoring relationship. 5. Participate in mid-year and end of year evaluations of the Mentorship Program. 

Note: You will get more information on all of these requirements once you receive your welcome letter into the program.
